LAPTOP REGISTRATION PAGE

Any UVI student wishing to use their laptop on the UVI Network must provide the following information and obey all Acceptable Use rules. Violation will result in losing Network privileges . This service is provided solely for your convenience in performing UVI Tasks and assignments.

Students must register their laptop with Information Technology, providing the following information:

Full Name of Owner

Name of Computer

Mac Address >Start>Run>Command> ipconfig /all at dos prompt) 00:00:00:00:00

Valid E-mail address

Contact Phone Number

I am an student enrolled at UVI Yes No

Operating System: Windows 9X Windows 2000 Windows XP Mac Linux

I have up-to-date virus software on my machine Yes Name of Software:

I have read and agree to UVI's Acceptable use Policy Yes UVI's Acceptable Use Policy

My machine is patched for the Blaster / Nachi Virus Yes! MS Blaster Patch

I confirm that all information supplied above is current and true.

I understand that if any information given above is false , I will be denied laptop access to UVI's Network



Guidelines & Policies:

 

        Violation of these rules or of UVI's Acceptable Use Policy  automatically and immediately will result in removal of user access rights and discipline which may include legal action. 

Other Microsoft updates and service packs are recommended.  A list of Windows software and current service packs can be located at Microsoft Update Page Microsoft Updates

 

Free Virus Software is available on the Internet - it is your responsibility to select, obtain, and maintain this software.  An example can be found at:

Machines found infected with or spreading virus' will have their access revoked until proof is given that the virus is removed and appropriate virus software is installed. 

These rules & guidelines are set to allow equitable, shared access to any eligible user to UVI's network.  Please be a responsible, cooperative network citizen and help us use and allocate these resources for teaching and learning.

UVI is not responsible for any damage incurred to personal equipment.

UVI reserves the right to deny network access to anyone at anytime
